The Heart Foundation’s Heart Health Check Risk Assessment and My Healthy Heart Plan templates have been embedded in the MyGPMPtool (MGT) software app, which is available to GPs using Pen CS’s Topbar application.

The templates are aimed at streamlining the delivery of heart health checks and allow GPs and practice nurses to efficiently carry out absolute cardiovascular disease risk assessment and management using pre-populated patient data and automated CVD risk calculation.
