Cerner looking to adapt real-time readmission prevention model to Australia

Global EMR giant Cerner Corporation is looking at adapting a successful model it has developed with Chicago-based hospital group Advocate Health Care to reduce avoidable hospital readmissions in the Australian setting.

The model is based around automated, continuous calculations of risk scores for certain conditions based on real-time data from the existing electronic medical record (EMR) and the use of care plans for high-risk populations.
